Home Stretch – Transition Coach (Male)

MG Corporation is searching for a Home Stretch – Transition Coach to help MG Corporation deliver the Home Stretch program. This is a full-time, Kununurra-based, male identified role with a salary range from $75,000 to $85,000 plus super.

The Position

As the Transition Coach you will guide, mentor and support young people aged 17- 21 years to transition from out-of-home care to independent living and adulthood. You will report to the Home Stretch Coordinator.

This will include:

  •  Providing access to supports and resources that will be made available through an extension of care arrangement. 
  • Offering tailored support services using a culturally safe and strengths based approach to case management and case coordination
  • Working collaboratively with relevant stakeholders including the Department of Communities’ staff, foster families, carers, the young person’s kin, community, and people identified as significant in their lives.

Qualifications and Licences

  • Experience and skills working with young people
  • Satisfactory National Police Clearance 
  • Satisfactory Working with Children (WWC) Check 
  • Satisfactory Department of Communities (DoC) Departmental Check (Departmental Record Check & Criminal Record Check) 
  • Current Drivers Licence and the ability to travel in response to organisational needs. 
  • Current First Aid Certificate
